GCS Nursing and Health Services works with our schools and the Guilford County Department of Health and Human Services-Public Health School Nurses. There are 42 School Health Nurses who provide a broad range of services and care to our students. Each school nurse averages service to 2-3 schools per year.

School Health Information
Although the grace period for submitting immunizations has passed, the North Carolina state law still requires students to have immunizations completed by the time they return to school. Students in grades Kindergarten, 7th, and 12th are required to have health assessments and/or immunizations completed in order to attend in-person learning and participate in activities on campus.
You can contact your health care provider or the Guilford County Health Department at 336-641-3245 to schedule an appointment. If you have any questions, please contact your child’s school.

Authorization for Medication for a Student at School
If a student must take medicine during school hours, they must have an Authorization for Medication for a Student at School form completed by a physician. A separate form will need to be completed for each medication.

HEALTH ASSESSMENT FORMS
NC Health Assessment Transmittal forms are required for all students entering a North Carolina Public School for the first time. Please find the forms here.
CARE PLANS
Care Plans for students will need to be completed by a healthcare provider annually. Please find the Care Plans here.
